The new patient paperwork is filled out prior to your child's initial evaluation. Please print the new patient paperwork as well as the questionnaire and return them at least 24 hours prior to your child's appointment.

Patient History
The initial evaluation questionnaire is filled out prior to your child's initial evaluation. Please feel free to print the initial evaluation packet as well as the new patient paperwork and return them at least 48 hours prior to your child's appointment. This allows the therapist to review all data prior to their initial evaluation.

The annual evaluation questionnaire is to be completed every year at the time of your child's re-assessment.
Patient Updates
If your child has had a change in personal information such as address, insurance or primary care physician, please complete a patient update packet and return to the front desk.
